I conjure you, spirit, by the Father and the Son and the Holy Spirit, and by these five names of God: Agla, Than, Joth, Tetragramaton, Adonay. Go to your place predestined by God, and depart in an honest manner, and do not harm nor molest any creature. And whenever and wherever I shall call you, come without delay and state, in the name of the Father and of the Son and of the Holy Spirit. Amen. By the good angels of God, these:

A hand-drawn pentagram (five-pointed star) is inscribed within a double circle. In the center of the star is the word "Agla" positioned between two crosses and above a capital letter "T". Each point of the star contains magical names and crosses: the top point has a red cross; the top-left point contains "Tetra-grama-ton"; the bottom-left point contains "Tha-n"; the bottom-right point contains "Ado-nay"; and the top-right point contains "Jo-th". There are additional crosses and symbols within the circle and around the star's vertices. The names of various angels and spirits are written in two columns flanking the diagram.
